Use the form on this page to register or update your medical provider numbers with AHSA for Access Gap Cover (AGC).
You can use the online form at the bottom of this page to:
- Register your medical provider number(s)
- Update your existing registration details (e.g. bank account, email, phone number, contact person, or postal address).
You do not need to register provider numbers that are only used for outpatient services.
Before you start
Please make sure you have all the required documents and information ready. You will need:
- AHPRA number(s)
- medical specialty(s)
- provider number(s)
- a copy of your current and new Bank details if they are changing
- a copy of the redacted bank statement to upload
- a copy of your Medicare confirmation letter OR screenshot of PRODA profile for each provider number (to upload).
- Doctor’s mobile number and personal (non-work related) email address.

Having trouble with the online form? Common issues include:
- You have left the web page idle for 60 seconds or more and the application has timed out.
- You have not cleared your cache.
- You do not have the current version of your web browser.

What to expect after lodgement
- Forms can take up to 14 days to process.
- You will receive an email once your registration or changes have been confirmed.
- Your registration will be backdated 3 months from the date it is processed. You will receive an email to confirm when you can start claiming.
Already registered with Access Gap Cover?
If your provider numbers are already registered with Access Gap Cover, you do not need to re-register.
