When you submit a provider registration form, you need to share some key documents. It's important that these documents contain the right information.
Before you begin completing the form, please check that your documents meet the guidelines below. If your documents do not meet these requirements, the form may be rejected.
Redacted bank statement
A redacted bank statement is a statement downloaded from your online banking or a copy of an original statement.
It must reflect:
- The financial institution name/logo
- Account name
- BSB number
- Account number.
Personal transactions may be redacted (i.e. hidden or covered).
If you have just opened a new bank account and don’t yet have a statement, you may instead provide the confirmation letter you would have received from the bank when you opened the account. It should show the new bank account details.
Medicare confirmation letter
A Medicare confirmation letter is a letter you would have received from Medicare when you applied for your provider number.
To retrieve the Medicare confirmation letter for doctor provider numbers:
- Login to HPOS > Services > My programs > My details > Messages/Mailbox centre
- The doctor will have a message in their inbox with a copy of the letter when they created the provider number.

If you cannot obtain a Medicare confirmation letter, a Provider Digital Access (PRODA) screenshot may be submitted instead.
The PRODA screenshot must capture the entire screen to be accepted. The screenshot must include:
- Date and time stamps,
- URL address
- Health Professional Online Services (HPOS) logo
- All relevant provider number(s) and practice locations.
To access the PRODA website:
- Find the PRODA login page on the Services Australia website.
- Log in to view your Medicare registered provider numbers via Health Professional Online Services (HPOS).
Practitioner personal mobile
Practitioner's personal mobile phone number must be supplied. It must be personal to the practitioner, as it may be used by AHSA to authenticate and confirm future changes to registration information, to protect against fraud.
Practitioner personal (non-work) e-mail
The personal e-mail address of the practitioner must be supplied. It must be personal to the practitioner and not related to any particular company or business. AHSA will use this to authenticate and confirm future changes to registration information, to protect against fraud.
