Our new provider registration online form enables you to register your provider numbers with AHSA for Access Gap Cover (AGC).
If different practices have distinct bank details or billing contact information, please complete separate forms for each group of practices.
This will help streamline communication and ensure each change is processed accurately.

Submit separate forms for different billing arrangements
If your Practice Manager uses one or more provider numbers paid to your own bank account, and a different provider number is managed by a billing agent, please complete separate forms for each group of provider numbers.
Use the same form for new and updated bank details (if contact details match)
If you're setting up a new provider number with different bank details, and also need to update the bank details for existing provider numbers registered with AHSA, you can include both on the same form, provided all provider numbers share the same contact information and postal address.
New registration required for billing contact changes
If you are changing billing contacts (no change to bank details), please register the provider number(s) as a new registration and supply all the required information and documents.
